10. Patient-Reported Outcome-based self-management of patients

Summary

  • Self-management can empower patients by building competencies to manage the challenges associated with their disease and treatment.  
  • HCPs can provide self-management support to empower patients to actively and collaboratively participating in their care.
  • Self-management support strategies can incorporate PROMs and include general information on disease and treatment, tailored self-management guidance, social support, PRO-based self assessment/monitoring of health status and clinical consultations.
  • Self-management support requires fostering an active, trustful HCP-patient partnership which can be supported by incorporating PROMs.
