Self-management support and role of PROMs

Patients might not be aware of what they can do to contribute to their clinical care or may not realise how important their role is in it. As HCP, you can empower patients by providing appropriate self-management support. 

Self-management support aims to strengthen patients’ knowledge, skills, and confidence in their active role. It involves a shift in the HCP–patient relationship toward shared responsibility and collaborative partnership, using a range of techniques and tools that enable patients to actively participate in decision making and their care.
